Mounjaro KwikPen: the four-dose pen explained

The Mounjaro KwikPen is a multi-dose pen that holds four weekly doses of tirzepatide in a single device. It replaces the routine of one pen per injection with one pen per month, and it is now the format many patients across Europe are being supplied with.

It also works differently from the single-dose pen. Needles are not included, the pen must be primed before every injection, and it has its own storage rules once opened. This guide explains what the device is, how it differs from the format it replaces, and what the European Medicines Agency authorisation actually covers.

What is the Mounjaro KwikPen?

It is a disposable, multi-dose, single-patient-use pre-filled pen. Each pen contains four fixed doses of 0.6 ml, taken one per week, so a single device covers four weeks of treatment. After the fourth dose the pen is discarded, even if a small amount of solution remains inside.

The active substance is tirzepatide, the same molecule used in Mounjaro weight loss treatment, a dual GIP and GLP-1 receptor agonist. It works primarily by regulating appetite, increasing the sense of fullness after eating, reducing hunger and lowering food cravings, which helps reduce food intake. In type 2 diabetes it also lowers blood glucose, acting only when glucose levels are high.

The pen is injected subcutaneously into the abdomen or thigh, or into the upper arm if another person is administering it. Injection sites should be rotated with each dose. It can be taken at any time of day, with or without meals, on the same day each week.

Mounjaro KwikPen strengths

Six strengths are authorised in the European Union. Each describes the dose delivered per injection, not the total pen contents.

How it differs from the single-dose pen

This is where most confusion arises for patients switching format.

Needles are not supplied in the pack. That single detail causes more first-dose delays than anything else about the device. Confirm with your pharmacy that compatible pen needles have been supplied before your first injection is due.

Using the pen safely

Every Mounjaro KwikPen is supplied with Instructions for Use, and those instructions take precedence over any summary. In outline, the routine is:

• Wash your hands, then remove the pen cap and inspect the pen and label.
• Do not use it if the pen is damaged, or if the solution is cloudy, discoloured or contains particles.
• Wipe the red inner seal with an alcohol swab before attaching a needle.
• Attach a new needle for every injection, then prime the pen with the needle pointing upwards.
• Inject into a rotated site, then replace the outer needle shield and remove the needle.
• Place used needles in a sharps container immediately. Never put loose needles in household waste.

If you also use insulin, inject it separately and never mix the two in the same syringe. The two may be given in the same body region but not immediately next to each other.

Storage

Before first use, the pen is refrigerated. Once in use, a Mounjaro KwikPen can be kept unrefrigerated below 30 °C for up to 30 days, after which it must be discarded regardless of how many doses remain. That 30 day window is one of the practical advantages of the format for anyone travelling.

What tirzepatide is authorised for in the EU

The European Commission authorisation, based on the assessment of the European Medicines Agency, covers adults with insufficiently controlled type 2 diabetes mellitus, and separately weight loss and weight maintenance in adults, alongside a reduced-calorie diet and increased physical activity. Use in type 2 diabetes also extends to adolescents and children aged 10 and over. Safety and efficacy have not been established in children under 10, or under 18 for weight management.

The pen contains 5.4 mg of benzyl alcohol per 0.6 ml dose, which is relevant for a small number of patients and worth raising with your prescriber. Each dose contains less than 1 mmol of sodium, so it is essentially sodium free.

Mounjaro KwikPen availability across Europe

Tirzepatide holds a centralised marketing authorisation, meaning the same authorisation applies across all EU member states. Supply, pricing and reimbursement, however, are national decisions, which is why availability differs noticeably between countries.

• Italy. Regulated nationally by the Agenzia Italiana del Farmaco. Prescription status and reimbursement conditions are set at national level.
• Spain. Overseen by the Agencia Española de Medicamentos y Productos Sanitarios, with financing decisions taken separately from authorisation.
• Germany. Medicines information and pharmacovigilance sit with the Bundesinstitut für Arzneimittel und Medizinprodukte. Germany was among the first European markets where tirzepatide launched.
• France. The Agence nationale de sécurité du médicament et des produits de santé oversees national medicines safety, supply monitoring and pharmacovigilance reporting.
• Netherlands. The College ter Beoordeling van Geneesmiddelen maintains the national medicines register and prescribing information.

In every one of these countries, tirzepatide is a prescription-only medicine. It cannot lawfully be supplied over the counter or without a prescription, and products offered without one should be treated as unsafe regardless of how they are presented.

Legitimate supply follows the same route everywhere in the EU: a clinical assessment, a prescription issued by a registered doctor, and dispensing by a registered pharmacy. Switching to the Mounjaro KwikPen from single-dose pens

Patients moving between formats are not changing medicine. The active substance, the weekly schedule and the dose strength all stay the same. What changes is the handling.

Three things catch people out. The first is needles, which arrive separately and are easy to overlook in a delivery. The second is priming, a step the single-dose pen does not have, and skipping it can mean an incomplete dose. The third is dose counting, because four injections now come from one device rather than four, so the pen has to be tracked across a month rather than opened fresh each week.

Marking the date of first use directly on the pen label solves the third problem entirely, and makes the 30 day limit easy to apply without guesswork. If you are considering a change of format or dose, raise it during your clinical review rather than adjusting on your own.

Side effects

The most frequently reported effects are gastrointestinal: nausea, diarrhoea, vomiting, constipation and abdominal pain, typically most noticeable when starting treatment or after a dose increase, and usually easing as the body adapts.

More serious risks in the authorised product information include acute pancreatitis, gallbladder disease, acute kidney injury following dehydration, hypoglycaemia when combined with insulin or a sulfonylurea, serious hypersensitivity reactions, and diabetic retinopathy complications. Tirzepatide is contraindicated in anyone with a personal or family history of medullary thyroid carcinoma, or with multiple endocrine neoplasia syndrome type 2.

Because tirzepatide slows gastric emptying, tell any anaesthetist that you are taking it before a procedure involving general anaesthesia or deep sedation, as residual stomach contents raise aspiration risk.
