Quick summary

Type of medicine

Hormone replacement therapy (HRT) containing estradiol, a natural oestrogen.

Works by

Oestrogel contains estradiol, an oestrogen identical to that naturally produced by the ovaries. It works by restoring oestrogen levels in postmenopausal women, helping to relieve symptoms related to hormonal deficiency such as hot flushes, night sweats, vaginal dryness, and sleep disturbances.

Active ingredient

Estradiol

Effective within

You may notice an improvement in symptoms within the first few weeks, but it typically takes 8 to 12 weeks of consistent use to fully rejuvenate the vaginal lining and eliminate dryness or pain.

Generic?

Yes

Side effects

Vagifem is very well tolerated because the hormone dose is extremely low. Common: Mild discharge or irritation in the first few weeks. Rare: Headache or abdominal pain. Note: Unlike oral HRT, Vagifem does not carry the same risks regarding blood clots because it is not absorbed systematically into the whole body.

Medication details

Description

Vagifem is the original, clinically proven topical treatment for vaginal dryness and discomfort associated with menopause. Manufactured by Novo Nordisk, it is widely considered the "gold standard" in local hormone replacement therapy.

Each Vagifem tablet contains 10 micrograms of estradiol and comes in a unique, pre-loaded single-use applicator. This hygienic design ensures the tablet is placed correctly and comfortably every time. Because it treats the symptoms locally without significantly raising blood oestrogen levels, it is a safe and effective choice for long-term relief.

Ingredients

Active ingredient: Estradiol hemihydrate (10 micrograms). Excipients: Hypromellose, lactose monohydrate, maize starch, magnesium stearate. Film-coating: Hypromellose, macrogol 6000.

Side effects

Breast tenderness, bloating, nausea, headaches, mood changes, or light vaginal bleeding may occur.
Less commonly: fluid retention, skin rashes, weight gain, or irritation at the application site.
Contact a doctor immediately if you experience chest pain, shortness of breath, jaundice, or sudden swelling of the legs.

Warnings

Do not use Oestrogel if you have breast cancer, endometrial cancer, unexplained vaginal bleeding, a history of thromboembolic disease, or severe liver disease.

In women with an intact uterus, treatment must be combined with a progestogen for 12 to 14 days per 28-day cycle to reduce the risk of endometrial hyperplasia.

Contact your doctor immediately if you experience chest pain, shortness of breath, swelling of the legs, or jaundice.

Frequently Asked Questions

Does this medication enter my bloodstream?

Vagifem is a "local" treatment. While a tiny amount of oestrogen may be absorbed, the vast majority stays in the vaginal tissue where it is needed. This is why it is considered safer than tablets or patches for women who only have vaginal symptoms.

Can I have sex while using this?

Yes. Vagifem helps restore lubrication and elasticity, making intercourse more comfortable. It is best to insert the tablet after sexual activity (before going to sleep) so that it stays in place overnight.

Is there a difference between Vagifem and Vagirux?

Only in branding and packaging. Vagifem is the original brand by Novo Nordisk, while Vagirux is a generic version. Both contain the exact same active ingredient (10mcg estradiol) and work in the same way. Some women prefer Vagifem for its specific applicator design or simply because they are used to the brand.

Do I need to take Progesterone with this?

Generally, No. Because the dose is local and very low, it usually does not affect the lining of the womb. Therefore, most doctors do not require you to take additional progesterone (like Utrogestan) if you are only using Vagifem.

Is the applicator reusable?

No. One of the key benefits of Vagifem is that each tablet comes in its own sterile, disposable applicator. You use it once to insert the tablet and then throw the plastic applicator away. This ensures maximum hygiene.
