Giving yourself a weekly injection feels daunting the first time, and then quickly becomes routine. Knowing how to inject Mounjaro correctly matters for two reasons: it makes the process more comfortable, and it ensures you actually receive your full dose. This guide covers preparation, the five steps, where to inject, and how to confirm the dose went in. Mounjaro comes in different presentations, so always read the Instructions for Use supplied with your specific pen. Mounjaro is a prescription-only medicine and this article is informational. If you are starting treatment, the Mounjaro treatment page explains how the process works.
Before you start
Store: in the fridge at 2°C to 8°C, never frozen
Timing: once weekly, same day, any hour, with or without food
Needle: a new one every injection
Site: rotate every week
Confirm: check the dose window or plunger before removing the needle
Step 1: Gather your supplies and prepare
Lay everything out before you begin: your pen, alcohol swabs, a gauze pad, and a sharps container or a sturdy sealed plastic alternative.
Take the pen out of the fridge about 30 minutes beforehand. Injecting medicine at room temperature is noticeably more comfortable than injecting it cold. While you wait, check three things: the expiry date, that the pen is undamaged, and that the liquid is clear and colourless to slightly yellow. If anything looks wrong, do not use it. For full storage rules, see our Mounjaro storage guide.
Step 2: Choose and clean your injection site
Wash and dry your hands, then clean the chosen site with an alcohol swab and let it air dry completely. Do not blow on it to speed that up, since that reintroduces bacteria.
Step 3: Prepare the pen
Single-dose pens arrive with the needle already attached. Keep the pen locked and leave the grey base cap on until you are ready. Once in position, pull the cap straight off and do not put it back, as that can damage the needle.
KwikPens need a needle attached each time, and needles are not supplied in the pack. Pull off the grey cap, wipe the red inner seal with a separate alcohol swab, screw on a new needle, then remove the outer cover (keep it for disposal) and the inner shield.
If a KwikPen is new, prime it first. Turn the dose knob until you hear two clicks and the extended line appears in the dose window. Hold the pen needle-up, tap the cartridge to raise air bubbles, then push the button in until it stops and hold for a slow count of five. You should see "0" in the dose window and a drop of liquid at the needle tip. If no drop appears, repeat up to twice more, then change the needle and try once more before contacting your pharmacy.

Step 4: Inject your dose
This is the core of how to inject Mounjaro, and the technique differs by pen type.
Single-dose pen: place it flat against your skin, turn the lock ring to unlock, then press and hold the purple button for at least 10 seconds. You will hear two clicks: the first when the injection starts, the second when it finishes. Confirm completion by looking for the grey plunger in the medication window.
KwikPen: turn the dose knob until it stops and "1" appears in the dose window. That is one full dose. Do not count clicks, and never attempt a partial dose. Insert the needle at a 90-degree angle (no need to pinch the skin), press the button until it stops, and hold for a slow count of five. Then check that "0" is showing before withdrawing.
A small drop of blood afterwards is normal. Dab gently with gauze rather than rubbing, which can irritate the site.
If "0" does not appear, reinsert the needle at the same site and finish the injection. Never redial the dose, as that risks giving yourself more than prescribed. The same principle applies at every stage of treatment, including your second dose and each step up through the titration schedule.
Step 5: Dispose safely
Single-dose pens go straight into a sharps container once used. For a KwikPen, replace the outer needle cap, unscrew the needle into the sharps container, and put the grey cap back on the pen.
Never store a KwikPen with the needle still attached. Medicine can leak out, and air and bacteria can get in. Discard the pen after four doses or 30 days from first use, whichever comes first. Leftover liquid after the fourth dose is expected and must not be used.
Where to inject Mounjaro
Three sites are approved, and how to inject Mounjaro safely depends partly on choosing them well:
-
Abdomen: at least 5 cm from the navel, avoiding the waistline and hip bones.
-
Outer thigh: never the inner thigh.
-
Back of the upper arm: only when someone else is injecting for you.
Absorption is similar across all three, so pick whichever feels most comfortable. What matters more is rotating sites each week and injecting into healthy skin, avoiding cuts, bruises, scars and stretch marks. For a full breakdown of each site and a rotation plan, see our Mounjaro injections guide.
Frequently asked questions
Does injecting Mounjaro hurt?
Most people feel little or nothing. Letting the pen reach room temperature first and rotating sites both help.
How do I know the full dose went in?
On a single-dose pen, look for the grey plunger in the window. On a KwikPen, look for "0" in the dose window.
How long do I hold the pen against my skin?
At least 10 seconds for a single-dose pen, and a slow count of five for a KwikPen.
Can I reuse a needle?
No. Use a new needle every time to reduce infection and injection-site reactions.
What if I injected in the wrong place?
Contact your prescriber. Do not inject a second dose to compensate unless told to.
In summary
Learning how to inject Mounjaro comes down to five steps: prepare your supplies and let the pen warm up, clean and air-dry the site, set up the pen correctly, inject and confirm the dose landed, then dispose safely. Rotate sites weekly, use a fresh needle every time, and always check the window before withdrawing.
